What Information does Head Start Physio (HSP) collect and why?

HSP will collect your personal identity information (e.g. date of birth, email, home address, phone number). This is for the purposes of being able to contact you, to identify you for appointment bookings, billing and to pass on any treatment advice, where required. 

HSP collects confidential and sensitive personal data regarding your health such as your medical history and relevant information about your life style. This is required to allow us to deliver safe and appropriate assessment and treatment programmes; to maintain appropriate medical records and to evaluate your progress.

In accordance with General Data Protection Regulations (GDPR) Head Start Physio will gain your consent by completion of a registration and consent form before collecting and storing your details.

How is this information collected?

You provide Head Start Physio your information in the following ways:

  • Providing information to HSP by phone or email
  • Completion of Registration and Consent Forms
  • Completion of Covid Screening Forms
  • During assessment and treatment consultations
  • When you use or view our website via your browser’s cookies.

How is this information stored and for how long?

Head Start Physio operate a paperless storage system. All details, even when collected via handwritten forms or via letters from your Consultant are scanned and stored onto a secure and password protected database system (Physiodiary).

Head Start Physio is legally required to store electronic clinical notes for adults for 8 years after your last treatment. Once this period of time has expired your details will be deleted from the database.

We will not share your data with any third parties without your explicit consent, unless the law allows for them to access it.
